What I hated about SPN was that it took SO long for Dean to realize that "Sam" wasn't Sam. 45 minutes into the episode and he finally realizes it. Come on. Also, the 6 minutes of commercials they keep showing at the end before the final 2 minutes of the show drive me absolutely nuts.
